UnityPoint Health-Meriter’s home health care agency known as UnityPoint at Home has become part of Chartwell, UW Health’s home health care agency, according to a report from the Wisconsin State Journal. The transition included this week’s closure of home medical equipment operations of UnityPoint at Home.
