State officials are still reviewing nonprofit SSM Health’s sale of nursing homes in Madison, Baraboo, and Waupun to for-profit Complete Care of New Jersey, originally expected by Sept. 1, the Wisconsin State Journal reports. The Madison facility, St. Mary’s Care Center, in the meantime was fined $51,165 this year for violations mainly involving a resident’s death in October following her fall from a bed.
The nursing home — the largest of 18 in Dane County — in December received an “immediate jeopardy” citation, the most serious type, from inspectors after the death of the 106-year-old woman with Alzheimer’s disease who fell out of bed and fatally hit her head on the floor when a nursing assistant removed a fall mat and turned her back on the resident.
The 184-bed facility has had 31 care-related regulatory violations in the time since, and its former five-star rating from the federal government has dropped to two stars. The issues have occurred since SSM Health hired Health Dimensions Group (HDG), based in Minnesota, to manage the nursing home in April 2022. HDG also manages other statewide SSM Health nursing homes.
