UW Health, UnityPoint Health-Meriter, and Agrace Hospice & Palliative Care have formed an affiliation agreement for hospice and palliative care designed to serve end-of-life needs. The partnership will allow the organizations to offer patients and families compassionate, coordinated care throughout their health care experience.
According to a release, the agreement means the organizations will work more formally to build stronger systems of care for patients with serious or life-limiting conditions. Agrace will remain a wholly independent nonprofit organization committed to serving patients from all area health systems.
