Two construction companies have been fined by federal labor inspectors following a worker’s death earlier this year at Green Bay’s Lambeau Field, according to the Wisconsin State Journal.

A 27-year-old carpenter for Mavid Construction, Joshua Shaw, died in June when he was struck by a dumbwaiter car while working inside one of the stadium’s two video scoreboards — which were being replaced as part of a larger project.

The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) issued citations against Mavid and the project’s general contractor, Miron Construction, on Nov. 15. The citations call for fines of over $15,000 against Mavid and nearly $19,000 against Miron for exposing employees and subcontractors to “pinch-point hazards” associated with the dumbwaiter and failing to ensure employees and contractors were qualified to operate the device.

The companies may comply, request an informal conference with OSHA officials, or contest the findings.

Mavid said in a statement that it is working to set up a scholarship in Shaw’s name and is committed to workplace safety. Miron officials echoed the latter sentiment.
